Coroners, regulators, courts and inspectors have documented what happens when restraint, lone working or aggression at work goes wrong. We publish those findings as short, sourced case pages: what happened, what went wrong, and what it means for the people running the service.
Lone Worker Casebook
Home visits, live-in care, night shifts, hostel work and lone security, where one person was on their own.
Restraint Casebook
Restraint injuries and deaths, and the technique that caused them: prone holds, seated holds, seclusion, and the monitoring that failed.
Restrictive practices in education
Restraint, seclusion and isolation rooms in schools and colleges: inspectorate findings, SEND tribunals, civil claims and inquests.
Staff injuries through violence at work
Nurses, teachers, shop workers, drivers and custody staff injured by the people they serve, where the injury was recorded in a tribunal, claim or prosecution.
How a case gets on this site
- There is a document. An inquest, judgment, tribunal, inspection report or inquiry. Not a press rumour, not a campaign claim, not a social post.
- Evidence and limits. Every page states its confidence, what could not be verified, and where the source runs out, including when that weakens the story.
- No names we cannot justify. People who died or were harmed are named as the public record names them. Staff are named only where they were convicted. Children never.
- The method is the finding. We describe the hold, the duration and the position, because that is what caused the harm. We do not publish instructions.
